Studio Tours: Saturday Sept 26 and Sunday Sept 27, 11am to 4 pm

Each artist is asked to provide the gallery with the following information by Sept 12, 2009.
Please print out this form and mail it to the gallery, or open in Word and complete and return by email.  Late entries cannot be accepted.
